    Job Summary


    We are seeking Nurse Practitioners and Physician Assistants to provide medical services in collaboration with our primary care physicians. Join us in pursuit of high quality patient care to provide a full scope of primary care services including but not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance.





    Essential Duties and Responsibilities


    • Obtains and documents patient history as appropriate to chief complaint or reason for visit.
    • Conducts relevant physical exam and evaluates patient, documenting pertinent positive and negative findings.
    • Diagnoses and treats disease and manages chronic illness.
    • Prescribes medication in accordance with New York State and federal law.
    • Orders appropriate laboratory, radiological and special examinations or tests; interprets results.
    • Provides preventative health services, including health education and counseling.
    • Collaborates with other clinicians and staff members to ensure continuity of care of patients.
    • Performs other job-related duties as required. Staff educational programs and meetings.








    Education & Experience



     Bachelor’s Degree required, Master’s Degree preferred.

    • Graduate of a Physician Assistant program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) or graduate of a Nurse Practitioner education program registered by the New York State Education Department.
    • Experienced Physician Assistants or Nurse Practitioners highly desired.
    • Current NCCPA Board Certification from the National Commission on the Certification of Physician Assistants (NCCPA) or Certification as a Nurse Practitioner from a New York State approved accrediting body (ANCC) or be eligible for board certification.
    • Current and unrestricted licensed Physician Assistant or Nurse Practitioner by the New York State Department of Education or be eligible for licensure.
    • Must possess Drug Enforcement Agency (DEA) registration or be eligible for registration.
    • Must possess Medicare/Medicaid Enrollment or be eligible to enroll.
    • Experience working with diverse populations in an ambulatory healthcare setting.
    • Must possess strong communication skills both oral and written.
    • Comfortable with Electronic Medical Record (EMR) system and Microsoft Office.



    • Master's Degree
    • 1-3 years of related experience
    • Experience working with diverse populations in an ambulatory healthcare setting




    • Graduate of a Physician Assistant program accredited by the Accreditation Review Commission on Education for the Physician Assistant (ARC-PA) OR Graduate of a Nurse Practitioner education program registered by the NYS Education Department
    • Excellent verbal and written communication skills   
    • Current New York State Physician Assistant License OR Licensed and currently registered as a Registered Nurse by NYS
    • Current Board Certification by the National Commission on Certification of Physician Assistants (NCCPA) OR Current New York State Registration as Nurse Practitioner
    • Current Board Certification as an NP from a NYS approved accrediting body (FOR NURSE PRACTITIONER)
    • Be eligible for Medicare/Medicaid Enrollment
    • Pre-employment background check, health physical and drug screening


    Medical Office Hours of Operation

    • Monday-Wednesday      8:00AM – 6:00PM
    • Thursday                       8:00AM – 8:00PM
    • Friday                           8:00AM – 5:00PM
    • Saturday                       8:00AM – 1:00PM
