    The Quality Specialist position provides outreach to patients to support HEDIS, HCC, and other quality improvement activities. The quality specialist position also supports addition activities aimed at improving HEDIS, HCC, and other quality and care coordination activities. These include but are not limited to identifying information in the medical record, providing information to providers and other staff members, and presenting quality results and other information to office staff.








    • Outreaches patients to schedule appointments. This includes determining what barriers or obstacles exist that prohibit patient from making or keeping appointment and determine what resources may be needed or action that should be taken.
    • Reads, understands and applies HEDIS/HCC specification and/or policy as it relates to compliance and outreach.
    • Reviews patient’s charts as needed to determine compliance based on specification or process outlined. Determine next steps based on chart review findings.
    • Determines possible root cause to issue effecting patient compliance and present issue and possible solutions to management.
    • Conduct work essential to supporting outreach efforts, including but not limited to:
      • Collecting and verifying compliance data.
      •  Entering data
      • Summarizing findings
      • Recording the results of outreach
      • Reporting results to management
    • Educate providers and other staff on quality related activities (e.g. HEDIS,HCC).
    • Performs other job-related duties as required.




    • High School Diploma or GED.
    • A minimum of two years’ work experience in health care setting.
    • Computer proficiency with Microsoft Word, Excel, and PowerPoint.
    • Knowledge of HEDIS quality measures.



    • Bachelor's degree
    • A minimum of two years’ work experience in quality related activities.
    • Experience utilizing electronic medical records systems (such as EPIC).
    • Experience with HEDIS, HCC, PCMH.
    • Excellent interpersonal, communication, organization and writing skills with the ability to effectively prioritize work.
    • Commitment to enhancing member experience.
