    We are seeking a Registered Dietitian who will serve as a member of the interdisciplinary health team in the treatment and care of patients.

    Position Highlights:

    • Oversees day-to-day functions of nutrition counseling practice and diabetes self- management programs
    • Provides both individualized and group counseling for patients
    • Develops nutrition education materials including meal and snack ideas and recipes
    • Participates in ongoing training and orientation on E.H.R. system, medical home requirements, compliance, continuing education and team building
    • Performs other job-related duties as required
    • Spanish Speaking Candidates preferred

    Provides medical nutrition therapy for patients of all age ranges for a variety of nutrition-related health conditions.           

    Provides diabetes self-management education and training for diabetes patient population.              

    Assesses patient nutritional status, diet history, food habits and determines risk level/needs. Confers with provider as appropriate on effect of nutritional status.           

    Provides patient/family counseling on appropriate nutrition and develops/implements plan of care. Follows up via phone and office visits.          

    Educate patients and families about managing conditions.              

    Counsels patients on special diets and diet techniques to ensure proper preparation and nutritional intake.            

    Records nutritional assessment/plan summary for medical record. Maintain accurate and complete medical charts and supply of educational materials.       

    Assesses nutritional status of patient population using anthropometric measurements, laboratory indices and nutrition histories.              

    Counsel patients on an individual basis regarding nutrition and behavior changes for desired outcomes.          

    Develop nutrition education materials to be added to group's standard of practice.              

    Coordinates with support staff to remind patients of appointments and collect appropriate information prior to appointments.          

    Participate in ongoing training and orientation on HER system, medical home requirements, continuing education and team building. 

    Maintain working knowledge of current trends in nutrition and health issues. Provide training for staff and physicians when appropriate.    

    Conduct group sessions for weight loss and diabetes self-management education, including education on the planning and preparation of food at home for these patients.      

    Is punctual and maintains a satisfactory attendance record.           

    Demonstrates flexibility in the acceptance and completion of work assignments.   

    Ensures that all patients receive personalized, prompt attention and are treated with receptiveness, dignity and respect.           

    Recognizes and performs duties which need to be performed although not directly assigned.               

    Attends all meetings as directed by Director of Nutrition and Medical Director.       

    Demonstrates flexibility in the acceptance and completion of work assignments.   

    Completes all necessary forms in an accurate and timely manner. 

    Demonstrates good judgment, discretion and ability to problem solve.       

    Performs other job-related duties as required.      


    Bachelor’s Degree required

    Successful completion of CDR examination for registered dietician (RD) required




    3-5 years of related experience required

    1-2 years of management experience required

    Knowledge of nutritional/dietetic principles and applications and the use of nutrition care process required

    Knowledge of concepts and methods of patient education including teaching and educational aids. Effectively develop and conduct education programs, serve as a resource to health team required

    Knowledge of how to design and implement special diets specific to a health condition required

    Skill in nutritional principles through appropriate assessment of status/needs and development of care plans based on appropriate calorie levels and nutrient adequacy required

    Skill in understanding of nutritional impact on health status by developing appropriate diets for patients with diabetes, hypertension, hypercholesterolemia, adult and pediatric obesity, pre/post natal, renal disease, gestational diabetes, cancer, digestive disorders, vitamin/mineral deficiencies etc. required

    3 years as a dietician, 2 years’ experience in a health care setting preferred

    Bilingual English/Spanish preferred


    Certified dietician/nutritionist by the state of New York required.
